I can't see where you are specifying the IO and IRQ settings here?
There are four options in all: IO address, IRQ, 8-bit DMA channel
and 16-bit DMA channel.
If your card is plug and play, you need to use isapnptools to
configure it first. I think the 2.2 kernel can do it for you, also.
